    In the submitted documents below, please include the SM ASSESMENT FORM found here. You need to download the form, fill it out and then upload it along with the rest of your files below.

    Documents you need to submit:

    1. Valid license.

    2. Last three pages of logbook.

    3. Valid medical certificate.

    4. For EU citizens ID card (both sides) - For non EU citizens copy of a valid passport.
